Designated Maintainer authorisation form
Designated Maintainers can use Access Manager to maintain people's records. Use this form to gain authorisation to become one.
The form should be returned to the Digital Data and Technology Security & Identity Management Team, by scanning and emailing to [email protected]. To remove Designated Maintainers, email your request to [email protected].
